Some important information before you dive in:
Birth Information
In order to secure a booking, you must be able to provide your full name, birth date, birth place, and birth time. For clients under 18, parental consent is required.
Payment & Deposit
A non-refundable deposit of half the full price of your chosen birth chart service is due at the time of booking. The remaining balance must be paid within 24 hours before your scheduled appointment. If full payment is not received, I reserve the right to cancel or reschedule the appointment.
Privacy Assured
Your personal birth information and any information discussed in your reading is completely confidential and will never be shared with anyone. Your trust and privacy is a top priority, and an essential component to the type of safe and secure container that I provide for each and every client.

1. What type of reading you are requesting: Individual, Group, or Children's
2. For Individual or Children's Charts, please indicate what pricing tier you are requesting: Supported Rate($), Sustainability Rate($$), or The Pay-It-Forward Rate($$$.) Remember, I offer a certain amount of supported rate bookings each month, so please take advantage if needed!
3. Tell me little bit about yourself or why you are interested in booking a reading with me.
4. Please include any questions you may have for me.
5. Please tell me if you would prefer a response via text, phone call, or e-mail. If you prefer text or phone call, please also leave your phone number as well.
6. If you are interested in a full or partial trade for a service or product you provide, please also remember to add that here. I cannot promise trades, but am always open to exploring this option.
PLEASE NOTE: Filling out this form sends an inquiry, but it does not mean your reading is booked. All official bookings/scheduling will take place via phone or e-mail and will be considered confirmed once a deposit has been paid.
